I accept payment by credit, debit card or personal check at the end of each visit.  For out-of-pocket consults I will provide you with a receipt (Superbill) which you can submit to your insurance provider for reimbursement (not a guarantee of payment).

Medicare Plans (MD referral required)

• Traditional Medicare Part B (only covers Diabetes and Non-Dialysis Kidney Disease).  I will verify your eligibility and advise you of any co-pay or deductible payment that is required, prior to your appointment.

I do not accept Medicaid.  If Medicaid is your primary insurance, please seek the services of a contracted Medicaid provider.
